What If I Have a C-Section? Will I Still Need My Doula?

When people think of hiring a doula, they often picture a natural, unmedicated birth. But what about if you have a planned or unexpected cesarean birth? Do you still need a doula?

I say absolutely!! Here’s what you can expect from your doula if you have a c-section birth:

If you know ahead of time:

  • Preparation for the day. What to bring. What the procedure will be like. What your partner should wear. And what will happen after your birth.

If your birth changes to a cesarean during the birth process:

  • Processing the emotions and helping with the transition.
  • Preparing your partner while you are being prepped for surgery.
  • Meeting you in recovery to help with skin-to-skin, breastfeeding, and support for you and your partner.

Having a doula by your side during a C-section can be just as beneficial as during a vaginal birth.

Here are some of the things I have done with my doula clients during their cesarean births and none of them have told me that they wished I wasn’t there or that they didn’t need my support.

  • Held a woman while she cried and processed the fact that birth was changing drastically for her.
  • Prayed with a father while we waited for his wife to be prepped for surgery.
  • Held space for a couple who were making really hard decisions about having a cesarean or not.
  • Took pictures of a dad meeting his new baby for the first time while his wife was still in surgery.
  • Ran for water, ice chips, and juice and fed them to the new mom who had her arms full admiring her baby for the first time.
  • Helped position a new baby and hold it close to the breast for a mom who was feeling overwhelmed and weak after surgery.
  • A womb healing meditation during our first postpartum visit.
  • Helped a mom process emotions and a story that was really hard to tell after her unexpected second cesarean.

During the C-Section: Advocacy and Comfort

Even in the operating room, a doula plays a crucial role. Many hospitals allow one support person in the OR, which is typically the baby’s father or your primary partner. In St. John's this is the case, however, your doula is still allowed to join you in the recovery room to support you and your partner.

Your doula can assist by:

  • Providing calming techniques such as guided breathing, visualization, or Reiki to reduce stress and fear.
  • Help you advocate for your preferences (such as delayed cord clamping or skin-to-skin contact) to ensure your birth is as personalized and fulfilling as possible.
  • Offering reassurance and emotional grounding before you go into surgery so you don’t feel alone or overwhelmed in the surgical environment.

After the C-Section: Postpartum Recovery and Support

Recovery from a C-section is different from a vaginal birth, and having a doula can make a world of difference in those first few hours and days. Your doula can assist with:

  • Skin-to-skin bonding and breastfeeding support, ensuring you and your baby get the best start even if you’re recovering in the post-op area.
  • Helping your partner navigate newborn care so you can rest and heal.
  • Providing emotional reassurance as you process your birth experience, especially if your C-section was unexpected or difficult.
  • Offering postpartum guidance on healing, pain management, and ways to care for yourself while recovering from surgery.
